A collision between a passenger train and a truck in South Africa has left at least 18 dead and 260 injured.

The deadly crash occurred between Hennenman and Kroonstad in the country’s Free State at about 9:15 a.m. local time, according to Sky News. The train was travelling from Port Elizabeth to Johannesburg.

Mthuthuzeli Swartz, acting CEO of the Passenger Rail Agency, said the accident was caused by “human error.”
